The normalising principals of moral treatment, the non-restraint policy of John Conolly and his followers in many countries, the therapeutic use of work in moral treatment, and the rediscovery of work as therapeutic in the last decades of the twentieth century.

In 1946 the National Mental Health Act (Public Law 79–487) authorized the establishment of a National Institute of Mental Health (NIMH), which was charged with 1. conducting research into the causes, diagnosis, and treatment of mental illness 2. training personnel in matters related to mental health 3. assisting states in developing community-based services. With the assumption that the treatment of mental illness would be furthered most effectively through a strong program of research and personnel training, the NIMH was placed under the auspices of the National Institutes of Health (NIH).

Similarly, since the Programme of Assertive Community Treatment (Stein and Test 1980) approach to assertive outreach was published there have been over twenty replication studies worldwide. Randomised controlled trials within these studies have been subject to systematic review (Marshall and Lockwood 1998) as have other case management services (Marshall et al. 1997). Despite the sheer volume of studies, controversy still reigns around the therapeutic value and cost benefits of these approaches (Burns 1997; Marshall 1996).
